AMERICAN BOFFER ACTIVITY WAIVER AND RELEASE OF LIABILITY

I, the undersigned, hereby acknowledge that I have voluntarily chosen to participate in the activities offered by the American Boffer.

1. ASSUMPTION OF RISK: I understand that the activities offered by American Boffer may be physically demanding and may involve risks of injury. I voluntarily assume all risks associated with my participation in these activities.

2. RELEASE OF LIABILITY: I hereby release American Boffer, its officers, employees, volunteers, and agents, from any and all liability for any injuries, losses, or damages I may sustain as a result of my participation in the activities offered by American Boffer, to the fullest extent permitted by law.

3. PHOTOGRAPHIC RELEASE: I grant American Boffer and its authorized representatives permission to photograph and/or record me at American Boffer's activities, and to use the photographs and/or recordings for any lawful purpose, including publicity, marketing, and web content.

4. MEDICAL TREATMENT: I hereby give my consent to American Boffer to provide or arrange for any medical assistance it deems necessary in the event of an injury during the activities. I agree to be financially responsible for any costs related to such treatment.

5. PERSONAL PROPERTY: I acknowledge that American Boffer is not responsible for any loss or damage to my personal property that may occur as a result of my participation in these activities.

6. RULES AND REGULATIONS: I agree to abide by all rules and regulations set forth by American Boffer and acknowledge that failure to adhere to these rules may result in my expulsion from the activity.

7. INDEMNIFICATION: I agree to indemnify and hold American Boffer harmless from any claims, actions, suits, costs, expenses, damages, and liabilities including attorney’s fees as a result of this participation.

8. SEVERABILITY: I acknowledge that if any portion of this agreement is found to be void or unenforceable, the remaining portions shall remain in full force and effect.

9. ACKNOWLEDGMENT OF VOLUNTARY PARTICIPATION: I confirm that I am participating voluntarily in these activities and that I have the option not to participate if I so choose.

For participants under 18 years old:

I, the undersigned parent or legal guardian of the participant, hereby give my approval to this child’s participation in the activities of American Boffer. I assume all risks and hazards incidental to such participation in these activities, and I hereby waive, release, absolve, indemnify, and agree to hold harmless American Boffer and it’s associates, as provided above.
